To enter your team into a tournament at South Side Ball Park @ Davis Field, please complete the following form. You will receive a phone call from a park official within 24 hours to confirm your team is entered.  If you do not receive a call within 24 hours, please call our office at (405) 691-4076 to confirm your team is entered in the tournament.
